Salary:

The Nurse Supervisor is an experienced and compassionate leader dedicated to ensuring the highest quality of care for all patients. Also responsible for the supervision of team leads and direct care staff while ensuring each individual patient’s care plan is established and followed.

Minimum Qualifications:

Must be a graduate of an accredited Nurse Degree program and possess a current, unencumbered, active license to practice as a Registered Nurse or Licensed Practical Nurse in the State of Michigan

Bachelors or Associates of Science Degree in Nursing preferred

Must possess and maintain current CPR Certification.

Experience: Prefer at least two (2) years’ experience with the TBI and Dementia population or rehabilitation.

Current valid driver's license with no more than (3) points or a valid state ID.

Must be able to pass a drug test

Must be able to work 8-12 hour shifts including holidays and weekends and understands nursing services must be available 24 hours per day, 7 days per week.

Excellent communication skills and be able to communicate clearly both verbally and in writing.

Leadership style must promote learning as well as employee engagement and retention.

Understanding and knowledge of CARF Accreditation is a plus.

RESPONSIBILITIES

Provide strategic leadership for resident by working with team leads, case managers and supportive staff members to coordinate patient care.

Provide clinical care through the direct application of the nursing process; perform and document resident assessments, assist with Care Plans, provide and delegate hands-on clinical care as indicated by the care plan and evaluate resident outcomes.

Provide training and oversight of the medication management program to ensure staff compliance with Medication Protocols ensuring resident safety in the medication use process. Review meds as new patients are admitted, medication changes have orrcured, and monthly thereafter.

Collaborate with physicians, pharmacists, and other clinical providers to coordinate care and services for the patient.

Provide clinical training and education as needed to nurses, team leads, floor managers, case managers, care coordinators, others who provide resident care. Perform skill evaluation to develop and promote clinical capabilities of team members.

Track, trend and report clinical quality data to identify risk. Actively participate as a member of the community Quality Assurance and Performance Improvement committee. Lead clinical quality and process improvement initiatives within the community to mitigate risk and improve resident care outcomes.

Review documentation from supportive staff to ensure care plan and medication compliance, ADL reviews, etc.

Train clinical team members and is responsible for performance management, evaluation and engagement.
