Job Title: RN Case Manager

Location: Denver, CO

Shift: Monday to Friday, 8:00 AM - 5:00 PM

Salary: $70,000 - $80,000 per year

Client Information:

Our client is a home care agency offering Personal Care, Respite Care, In-Home Care, Speech Therapy, and Brain Injury Services.

Position Information:

The Case Manager RN provides skilled nursing care per the client's plan of treatment, including comprehensive health and psychosocial evaluation, monitoring of services, teaching and training activities, and provision of direct nursing care. Works under the supervision of Director of Clinical/Professional Services or designee. Exercises supervision of assigned personnel including licensed practical nurses, home health aides, and other support staff.

Additional Details:

Case Managers are expected to be available by telephone and in person to client/staff Monday to Friday, 8:00 a.m. to 5:00 p.m. If census is low, then Case Manager will work in the office. May require inclusion in on-call rotation, Monday to Friday after 5:00 p.m., and weekends depending on office location. Full-time, salaried Registered Nurse Case Manager will be expected to complete visits on a point system. Point system as follows:

Start of Care = 2 points

Resumption of Care = 1.5 points

Discharge = 1.5 points

Recertification = 1.5 points

Skilled Nurse visit = 1 point

Transfer = 0.5 points

HHA supervisory visit = 0.5 points

Brief visit = 0.5 points

Case Managers will be required to complete a minimum number of visits each week to equal 25-30 points. If the number of visits/points continually falls below this amount, the Case Manager will be changed to a per-visit rate. Any visits greater than 30 points per week will be compensated at a per-visit rate.

Requirements

ASN required, BSN preferred

Active state of Colorado RN license or compact

1 year of home care experience preferred

Valid driver's license

Valid automobile insurance

Access to a reliable vehicle
