Solutions In-Home Care is looking for Home Care Aides who align to our Core Values to join our team! You will be working directly with patients, following a one-on-one care plan in the patients home. You operate with a big heart, not only providing personalized and attentive care, but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets the patients needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health, Dementia or Hospice experience is a plus.

RESPONSIBILITIES
Assist patients with daily activities such as moving in and out of bed, dressing, bathing, wheelchair, or automobiles
Care for patients by dressing, bathing, changing bed linens, doing laundry, housekeeping, assisting with personal care, plan/prepare meals/snacks
Maintain records of patient care, condition, progress, or problems to report and discuss observations with supervisor
Build relationships with patients by conversing, games, or reading aloud to help keep them mentally healthy and alert
Give medication reminders QUALIFICATIONS
Valid Home Care Aide (HCA) certification
Instructor Lead CPR and First aid certification
Food Handlers Card
Ability to adhere to all health and safety guidelines
Excellent communication and interpersonal skills
Preferred Valid drivers license and reliable transportation
