As a BILINGUAL SPANISH Diabetes Care Coordinator you will be responsible for supporting pharmacists and educators in clinical programs offered to patients for ongoing diabetes self-care management.

Alaska, California, Colorado, Delaware, Hawaii, Idaho, Maine, Montana, Nevada, New Hampshire, New York, North Dakota, Rhode Island, South Dakota, Vermont, Washington, Wyoming; residents are not eligible.

Responsibilities:

Performs inbound and outbound patient enrollment calls to diabetes wellness program along with continued support after initial delivery of medical equipment and/or supplies

Proficient knowledge regarding diabetes disease state and all associated medical equipment and supplies

Ability to guide a patient through how to use their medical equipment and the key-features

Ability to trouble shoot issues by telephone that may occur with medical equipment

Ability to access if patient has additional needs for on-going diabetes education and coaching

Assist pharmacists and certified diabetes educators with patient survey questions regarding diabetes disease management

Performs administrative functions such as scheduling delivery of medical supplies

Responds to patient service-related questions and/or concerns

Achieves stated goals and objectives for patient engagement

Perform meter training sessions and access additional needs for on-going diabetes training and support by telephone for diabetic patients

Proficient in training diabetic patients on using blood glucose monitor

Assist pharmacists and certified diabetes educators with patient survey questions regarding diabetes disease management.

Achieves stated goals and objectives as outlined by managements

Possesses a strong pharmacy knowledge base including translation of sig codes, product selection, basic knowledge of brand and generic drugs by name

Maintains a complete understanding of prescription processing including: data entry, creation of shipment schedules, verification of insurance and patient eligibility, prescriber calls for prescription refills, intake of refill Rxs from prescriber, generation and collation of processed queues

Answers inbound patient-related calls, when needed

Maintains a working knowledge of assigned specific pharmacy area/functions as well as cross training in other areas throughout the pharmacy as indicated by management

Contacts doctors’ offices to request new prescriptions for pharmacy patients

Courteously assists all customers, internal and external

Maintains a high degree of confidentiality at all times due to access to sensitive information

Maintains regular, predictable, consistent attendance and is flexible to meet the needs of the department

Follows all Medicare, Medicaid, HIPAA, and Private Insurance regulations and requirements

Abides by all regulations, policies, procedures and standards

Qualifications:

High School diploma or GED with a minimum 2+ years working as a Pharmacy Technician (inbound/Outbound call center experience preferred) required

Must maintain in good standing registration as a Pharmacy Technician with the state

Certified Pharmacy Technician is preferred

Maintain all required continuing education for licensure and certification

Prefer basic understanding of diabetes, diabetic care, diabetic medications, glucometer management, finger stick blood sugar testing, insulin administration and be able to recognize and act upon symptoms of hypoglycemia and hyperglycemia as indicated per State regulations.

Will be required complete AADE training for a paraprofessional level II and complete ongoing training as directed by supervisor

Must have compassion for and desire to work patients. Must have basic computer skills or willing to learn computer skills needed for e-MAR system.
