Position Description:

Employees in this position perform duties related to the education and support of program clients. Employees in this position are aware of available community resources, they make appropriate referrals to same, and they function as a member of the program team.

Position Expectation:

Primary Duties and Responsibilities:

Assesses participants for the purposes of services and education needed.

Participates in individual or group education of program participants.

Acts as a resource for program participants and other staff members.

Performs case management functions such as maintaining case records and coordinating additional services as needed.

Acts as a group leader/facilitator during group education sessions.

Collects demographic information from participants. Maintains records on interactions with participants.

Makes contact and promotes the program with outside agencies, senior centers, churches, retirement communities, assisted living facilities, and other agencies or facilities who cater to individuals aged 55 and over.

Represents Tri-County in a positive way in the community and with program participants.

Performs other reasonable and related duties as requested by the immediate supervisor or administrative personnel.

Minimum Training and Experience:

Requires Bachelor's Degree in field related to Human Services from an accredited college or university, which include but not limited to: Social Work; Clinical Psychology; Counseling; Rehabilitative Counseling; Counseling Education; Education; Criminal Justice; or Ministry.

Requires participation in a staff training program.

Prefer considerable knowledge of community resources. Required ability to research and find community resources and information.
