Hospice Chaplain-Part Time

The Parker Jewish Institute, located in New Hyde Park, New York, is seeking a dedicated and experienced Chaplain with prior Hospice and Spiritual Coordination experience, to join our team in Hospice, to provide spiritual care to our Hospice patients, their loved ones, and as appropriate, using an inter-faith model of care and working within the patient's belief and value system.

This position that will include field visits in Queens and Nassau counties in addition to in-patient visits.

Job Responsibilities Include:

Conducts spiritual assessment of patients and families/significant others, and develops a plan for meeting needs

Provides spiritual counseling interventions with patients, families, and significant others

Participates with Interdisciplinary Team to develop comprehensive patient/family care plan that is designed to meet the holistic needs of Hospice patients and families

Assists the Hospice Team with ethical issues regarding patient and/or family situations by facilitating discussions at IDT meetings and providing education to the team on ethical concerns

Assists with bereavement counseling, working collaboratively with the Bereavement Coordinator

Assists patients and /or families to participate in meaningful spiritual practices and rituals, as requested by patients and families

Documents assessments, interventions and patient/family outcomes in the EMR

Position Qualifications:

College Degree in a related field

Minimum of one (1) year of training and experience in pastoral/spiritual counseling

Previous Hospice experience strongly preferred

Valid New York State driver's license and able to make field visits in Queens and Nassau counties

Fully Vaccinated against COVID-19
