General Description:

Advanced practice provider (Physician Assistant or Nurse Practitioner) who can lead and develop a collaborative, multidisciplinary wound care team in multiple locations daily. The provider will travel to assigned/scheduled locations daily for clinical wound rounds at various facilities within a designated region to provide direct patient care to patients with compromised skin integrity. They will be responsible for ordering, interpreting, and evaluating laboratory and diagnostic studies to adjust treatment to patients changing medical condition and to assist with wound healing. They will perform bedside wound care in each of the assigned facilities and in the outpatient office setting and provide timely documentation of these visits. The provider will professionally collaborate with all members of the patient’s care team to execute the shared vision of the Premier Wound Solutions team to provide the absolute best wound care possible.

Responsibilities:

Board certified Physician Assistant or Nurse practitioner (NP/FNP/ARNP/APRN) licensed in designated state

Active DEA licensure in good standing

Wound care experience is preferred but not required; on the job training and ongoing clinical education will be included in this position

Responsible for providing direct and indirect patient care that includes consultations, assessment, diagnosis, education, planning and treatment of wounds, skin, and ostomies. Wound care skills: wound assessment and staging if necessary, dressing application, obtaining molecular wound cultures, and amniotic grafting on clinically appropriate patients, minor surgical procedures including but not limited to surgical debridement and suturing, uncomplicated G tube and suprapubic catheter replacement, offloading treatment/management, and pain control.

Evaluate the effectiveness of care provided and revise care plan, as necessary

Identify patient care situations which require urgent intervention and assist with timely care coordination of these patients

Serve as a consultant to nurses and healthcare professionals in managing complex patient care scenarios.

Assist in orienting, training, and evaluating staff on issues pertaining to wound care, resident care, safety, job duties and communication skills

Identify and be responsible for self-development

Driving is required. Must have reliable transportation, a valid driver’s license, and auto insurance in good standing

Must be a motivated self-starter who is reliable, punctual, and able to work and schedule appointments independently

Must have strong interpersonal skills and the ability to communicate effectively verbally and in written form.

Must be able to provide accurate and timely documentation of each patient encounter. Documentation will be completed and signed within 24 hours of patient visit

Assist in orienting, training, and evaluating staff on issues pertaining to resident/patient care, safety, job duties and communication skills.

Qualifications:

Bachelor’s or Master’s degree from accredited Physician Assistant or Nurse Practitioner program, board certification, state licensure in designated state
