Job overview

Ventracare Homes Inc is a transitional home for medically fragile children located in Chicago Ridge, Illinois. Our job is to provide skilled transitional and respite care services to medically fragile children and their families. To give parents the gift of time to have personal time and experience fun activities, knowing that their children are in good hands. W need a friendly and compassionate staff to represent our organization as well as develop new relationships and grow our business.

We are looking for 6 full time LPNs and 2 RNs to join our company.

Schedule: Sunday to Saturday. 12 hour shifts 7 am to 7 pm and 7pm to 7 am

Pay: LPN rate $35/hr. RN rate - $45/hr

Patient/Nurse ratio: 4 Patients:1 Nurse and 1 CNA

The nurses deliver direct and indirect care to patients from infancy to some young adults that are medically fragile and ventilator dependent and chronic medical conditions. The LPN functions under the direction of a professional Registered Nurse.

Responsibilities

· Patient observation and implementing care to meet identified needs of patients and families based on the plan of care.

· Uses equipment and supplies effectively and efficiently such as ventilator, tracheostomy, HME, G tube, suction machine, humidifier, infusion machine etc.

· Utilizing knowledge of growth and development in delivery of patient care.

· Teaching patients and families according to plan devised by physician, Professional Nurse/ and Nurse Case Manager.

· Administering medications to patients under the supervision of a Registered Nurse. (The L.P.N. must meet the established criteria of the Medication Performance Evaluation Tool within the 3-month probation period.)

· Maintaining a clean, safe environment for the pediatric patient.

· Communicates the condition of the patient to appropriate members of the

health care team.

· Reporting information to Care Team Leader, Charge Nurse

· Documenting significant information in medical records.

· Evaluating care plan and the patient's response to the plan in conjunction with primary team and modifying the plan based on patient and family needs.

· Participating in care team conferences

· Maintains required job skills and professional competencies.

· Attends required educational sessions.

Qualifications

Active state nursing license in Illinois

Pediatric experience is required; Pediatric ER, Pediatric hospital unit, or Pediatrics Urgent Care Experience preferred

Prior experience and competency with nursing procedures (not limited to IV placement, urinary straight catheterization, and medication administration)

Current Basic Life Support (BLS) is required, Pediatric Advanced Life Support (PALS within 30 days of hire)

Knowledge of medical practice and terminology, clinic’s processes, and procedures

Knowledge of medical equipment and instruments

Skill in rapid assessment of the critical presentation of the pediatric population

Ability to use sound reasoning and judgment and act calm in emergencies

Ability to effectively communicate both verbally and nonverbally

Ventracare Homes, Inc. is a welcoming and safe environment where children in transition can overcome the barriers they face in preparing to go home after hospital admissions. Ventracare is a 12-bed Community-Based Children’s Health Care Facility that provides nursing care, respite, or transitional care to children younger than 22 years of age. The care period for respite is 1 to 14 days, and 1 to 120 days for transitional care to home or other appropriate settings for medically fragile children, technology-dependent children, and children with special health care needs who are deemed clinically stable by a physician.
