Case Manager II - MCC

Job Description

The Case Manager II - Medical Care Coordination (MCC) will facilitate patient intake and case management of HIV-positive patients. The position works with individuals in the clinic and other settings as well.

Reports to: MCC Manager/HIV Program Administrator

Qualifications:

1. Case Manager II: Must have a bachelor’s degree and HIV knowledge.

2. Must be able to relate to a variety of individuals from all walks of life, including ethnicity, gender, sexual orientation, age, and diverse backgrounds. Ability to work with populations impacted by HIV (gay males, IDUS, etc.).

3. Experience and knowledge of Casewatch are preferred.

4. Effective verbal and written communication skills to communicate clearly and effectively with patients and others.

5. Sensitivity to the different cultures represented among members and staff.

6. Demonstrated ability to listen and communicate with others in a professional and caring manner, including sensitivity to individuals from diverse cultures and lifestyles.

7. Demonstrated ability to set priorities for tasks to work effectively in spite of interruptions and under minimal supervision. Self-starter, reliable and dependable.

8. Fluency in English (speaks, read and write).

9. Fluency in Spanish (speak, translate, read, and write) is required.

10. Ability to solve problems and make routine recommendations.

11. Ability to maintain absolute confidentiality about health care and other patient/client information.

12. Current California driver’s license, appropriate insurance coverage, and a driving record acceptable to the NEVHC’s insurance carrier (if required to drive on the job).

13. The MIS Patient Information System access level is 2.
