GENERAL SUMMARY:

Comprehensive Community Services (CCS) is a recovery-focused, integrated behavioral health program for adults with severe mental illness &/or substance use disorders & children with severe emotional disturbance. CCS is unique for its inclusion of both children & adults & its focus on other physical illness & impact on multiple system use. CCS is a community-based program in which the majority of services are provided in clients’ homes & communities. The program is person-centered & uses client-directed service plans to describe the individualized services that will support the client to achieve their recovery goals. Services are provided by teams of professionals, peer specialists, & natural supports, all coordinated by a CCS service facilitator.

The MHP is responsible for the authorization of CCS services per DHS 36.15. This includes reviewing and attesting to the consumers need for psychosocial rehabilitation services and medical and supportive activities to address the desired recover goals.

ESSENTIAL FUNCTIONS:

Adheres to St. Charles Youth & Family Services Policy & Procedures.

Integrity & quality of programs & services to clients through quality improvement, accreditation, licensing & internal monitoring processes.

Ensures equitable treatment of staff through consistent application of Agency Human Resource processes.

Ensures mitigation of risk through risk management analysis & corrective action initiatives.

Provides direct &/or Clinical supervision to staff as required by contract.

Plans, implements, monitors & evaluates program initiatives that support the agency mission & vision.

Ensures accurate & timely dissemination of information to program staff.

Cultivates & maintains relationship with community/contractual partners.

Ensures timely enrollment & service provision of referrals of eligible consumers

Participates in agency-wide & contract required committees, trainings & meetings.

Compiles & reports in a timely & accurate manner program statistics at designated intervals.

Conducts weekly meetings & individual staffing with program staff as required by contract.

Serves as a member of agency management.

Supports the goals, mission & vision of the agency & actively works to implement agency strategic planning.

Ensures timely completion of program –related documentation & ensures timely submission to contractor as requested.

Represents program at various community functions.

Knowledge of Milwaukee County community and mental health resources and services

Knowledge of DHS 36

Ability to complete a comprehensive assessment with youth and family and provide applicable diagnosis for youth

Assist case managers with obtaining diagnosis from current outpatient providers

Available for additional meetings with families/psychoeducation (clinical information on diagnosis, comorbidity, prognosis, treatment) – per families request

All other duties as assigned.

MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:

Licensed by the state of Wisconsin as LCSW, LPC, LMFT, etc. highly preferred. Meets the requirements as defined in DHS 36.10 (2) (g) 1 to 8.

Experience working with targeted Recovery Support populations, i.e. CARS, CCS, RSC, TCM, CSP etc. highly preferred.

Ability to prepare formal reports using proper writing skills.

Strong organizational skills, ability to manage multiple complex tasks, strong professional communication & team work.

Ability to accurately perform record keeping & documentation entries.

Working knowledge of computer applications.

Supervisory or leadership experience highly preferred.

Knowledge base of Mental Health & AODA service provision & community resources.

Ability to interpret & implement statues related to the program.

Strong verbal & written communication skills.

Ability to display cultural competence by responding respectfully & effectively to people of all cultures, languages, classes, races, ethnic backgrounds, religions & other diversity factors in a manner that recognizes, affirms & values the worth of individuals, families & communities & protects & preserves the dignity of each.

Ability to use vehicles for work related duties according to the agency’s insurance liability standards.

Valid Wisconsin driver’s license & liability insurance.

Ability to display cultural competence and sensitivity to the populations we service by responding respectfully and effectively to people of all cultures, socioeconomic characteristics, languages, classes, races, ethnic backgrounds, religions, and other diversity factors.

WORKING CONDITIONS

General Office environment/multiple locations to include community sites.

In-Home or Community home settings during client visits.

Local travel as required fulfilling essential functions of the position.

Ability to lift up to 30 lbs.

Potential for exposure to blood products, contagious disease, etc.
