DESCRIPTION:

Responsible for case management by triaging patients referred to the MAT program, conducting nurse intake visits, chart reviews, Suboxone inductions, nurse visits for Vivitrol, collaborate with community partners for program access, schedule new patient referrals, monitor buprenorphine patient panels, working with care coordinator for scheduling medical appointments, and ensure prescribing providers are scheduled with patients.

Works closely with Lead Physician, BH provider, and Clinical Director to grow the program and provide evidence-based patient care. Provides supervision to medical assistant and care coordinator, working closely with them as a team to ensure patient needs are met. Work with providers to evaluate the plan of care and ensures continued appropriateness based on the patients changing needs and condition. Maintains working knowledge of specialized case and utilization management methodologies and practices.

Performs other duties as needed.

Schedule: 4 days per week, 10 hours per day

ESSENTIAL FUNCTIONS AND DELIVERABLES:

Reviews the patient's electronic medical record to provide quality care through developing, implementing, and evaluating individual patient and formulates with the provider a plan of care appropriate for the patient.

Communicates with other case health professionals and coordinators with regard to patient's situation. Facilitates and ensure recommendations are communicated across the healthcare team. Will collaborate very closely with Behavioral Health team member to ensure patients willingness to participate in recovery.

Collaborates with physicians, other health care professionals (therapists, social services, pastoral care, and supportive services), clients, and families in developing a comprehensive, coordinated plan for care. Make appropriate request to Medical Provider when medication dosage change is indicated and carry out those orders in an accurate and timely manner.

Assists with the development of goals and objectives of the SACHS clinics as it pertains continuum of patient care. May assist with triaging patients when necessary.

Monitor and promote effective utilization of healthcare resources through EMR and clinical measures. Tracks medical and behavioral health outcomes of the patients.

Responsible for reviewing patient's prescriptions and OTC medications to ensure they are properly prescribed and to assess whether client is aware of adverse reactions. Carry out all physician orders including telephone orders.

Maintains working knowledge of specialized case and utilization management methodologies and practices; applies concepts to every-day practice.

Works with all levels of staff to ensure SACHS program measures are being met. Providing pharmaceutical counseling/education to patients and staff. Consulting with clinic physicians and staff on pharmacology/pharmacokinetic questions concerning agency patients.

Assist develop policies/procedures, clinical guidelines, ancillary guidelines, etc., for patient care; reviews and revises as indicated.

Communicates with other case health professionals and coordinators with regard to patient's situation. Facilitates and ensure recommendations are communicated across the healthcare team. Will collaborate very closely with Behavioral Health team member to ensure patients willingness to participate in recovery.

Assist with controlled substance medication to monitor for overuse/abuse when deemed necessary by the pharmacist, nurse, or physician. Responsible for assisting patient with receiving authorization or benefits needed to assist the patient while in the BH clinic.

Maintains current knowledge of all documents that direct practice, e.g., compliance policies, clinical guidelines, ancillary guidelines, etc.

QUALIFICATIONS:

Education: Satisfactory completion of a Nursing program; Bachelor's in Nursing (BSN) degree preferred.

Licensure/Certification: Current California Registered Nursing License, current CPR/BLS certification, valid California driver's license and liability insurance. As a requirement of this position you must receive EPIC certification for the module you have been hired into.

Experience: Minimum of two (2) years case management in a behavioral health or substance use setting, Medicated Assisted Therapy (MAT) experience preferred. Outpatient clinic preferred or equivalent at a supervisory level. Bilingual in English and Spanish preferred.

Essential Technical/Motor Skills: Computer competency; Able to read; write legibly; speak in English with professional quality; use computer, printer, and software programs necessary to the position.

Interpersonal Skills: Able to relate and communicate positively, effectively, and professionally with others; be assertive and consistent in following and/or enforcing policies; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate; accept direction. Able to communicate effectively in English in person, in writing, and on the telephone; think critically; perform basic math functions; manage multiple assignments effectively; compose written material; organize and prioritize workload; work well under pressure; problem solve; recall information with accuracy; pay close attention to detail; work independently with minimal supervision.

Other: Must maintain a professional appearance along with friendly, cooperative, caring and calm behavior. Must be committed to serve low-income clients.

Must be legally authorized to work in the United States on a full-time basis. Must not now or in the future require sponsorship for employment visas.
