A social services agency in New York City is looking to add a Spanish-speaking professional to join their growing Comprehensive Program as a Case Manager. Working under the Program Supervisor, the Case Manager will be responsible for both clerical and practical support.
Responsibilities:
The Case Manager will:
Enroll 50 participants into the program and lead and manage 50 cases independently
Conduct timely intake, orientation, and assessment of all participants in order to develop service plans to help their families gain confidence, self-sufficiency, and greater connection to their community
Provide advocacy, outreach, and referrals to entitlement agencies, social service providers, and other programs on behalf of program participants
Perform related crisis intervention activities and escort participants to appointments as needed
Develop ISP plan with short- and long-term goals for participants enrolled in the program
Develop self-advocacy goals with participants enrolled in the program
Perform follow-ups on bi-weekly basis.
Qualifications:
Bachelor's Degree in Human Services or relevant field required or completion of the Family Development Credential (FDC) Program within the first 6 months of employment
Bilingual (English and Spanish)
Great interpersonal skills
Excellent communication skills (written and verbal)
Desired Skills:
1+ year of Case Management experience in a Community-based setting
Counseling credentials
