We are looking for an experienced Clinical Documentation Specialist to join our Care/Quality Team. As part of the Care Team, this role will have a strong focus on preparing charts for Providers. This position also assists with managing patient care by monitoring treatment plans, educating patients on their conditions, and connecting them with healthcare providers.

Essential Duties and Responsibilities:

Prepare and assemble medical record charting and documentation for the Provider.

Maintain all data and process controls to ensure compliance.

Recording patient's progress including charting referrals, home visits, and other notable interactions

Communicate patient needs to a variety of care team members and follow up accordingly

Manage discharge plans upon completion of treatment

Work collaboratively with patients, families, physicians, and nurses to ensure high-quality care

Coordinate and facilitate patient care through assessment, evaluation, planning, and implementation

Act as the patient's advocate as it relates to insurance coverage and financial assistance

Maintain the patient's comprehensive clinical record through detailed documentation

Coordinate with third-party payors on a regular basis

The overall goal is to improve clinical outcomes, increase patient satisfaction and promote cost-effectiveness

Additional duties assigned by management

Qualifications/Education:

Bachelors degree in Healthcare or related field is preferred

Certified Medical Assistant, or previous experience, is strongly preferred

Bilingual in English and Spanish is required for this clinic location

4+ years experience prepping charts and completing chart audits

Knowledge of chronic diseases, such as COPD/Asthma and, Diabetes

EMR experience is required, eClinicalWorks (eCW) is a plus

Experience with preparing charts and validation of ICD coding

Proficient computer skills including experience with Microsoft Excel

Strong written and verbal communication skills

Excellent written and verbal communication skills
