Job Summary:
The Licensed Vocational Nurse is responsible for providing direct patient care under the supervision of a Registered Nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.
Qualifications:
· Licensed Vocational Nurse, per the state requirements.
· Good communication skills to be able to relate professionally and positively to patients, caregivers, colleagues and physicians and strong clinical documentation skills.
· Ability to read, write, and speak the English language.

Essential functions:
· Under the direction of the RN, assist in identifying the patient’s physical, psychosocial, and environmental needs.
· Participate in planning and implementing care in conjunction with the RN, in accordance with the POC.
· Provide care utilizing infection control measures that protect both staff and patient (OSHA).
· Records treatments administered in medical record.
· Records clinical documentation according to accepted regulatory, corporate and professional guidelines.
· Participates in weekly case conferences.
· Participates in interdisciplinary communication to ensure that patients’ needs and expectation for services provided are understood and to ensure optimization of service delivery and patient progress.
· Provides verbal or written patient status updates to supervising RN prior to reassessment, supervisory and/or discharge visits, and as indicated by patient response to treatment.
· Participates in in-service training programs.
· Responsible for assuring patient safety.
· Complies with state regulations and national standards
