Job Description

The Case Manager implements the individual and family case management services program. The Case Manager ensures that each client can participate fully in relevant and appropriate social, academic, and independent living activities. The Case Manager advises and counsels the clients, refers agency workers to the activities schedule, and directs case management services. The Case Manager collaborates with clinical personnel, businesses, schools, and parents on programmatic partnerships. The Case Manager will provide primary responsibility for the quality assurance of services provided to the clients. In addition, the Case Manager will lead client intake: assessment, assignments, treatment plan development, and documentation: discharge summaries and post-release planning.

Direct Reports:

The Case Manager reports directly to the Client Services Coordinator

Essential Functions:

1. Case Management Services:

Ensure all clients are meeting at a minimum all standard requirements of their referring agency (e.g., staying drug-free,maintaining daily appropriate hygiene, working and or attending school full-time, volunteering)

Meet weekly and as needed with clients (individuals and families) on caseload to develop case plans, review progress, and follow up on tasks and services.

Assist in planning monthly extra-curricular or therapeutic activities for a client base.

Lead in the development of daily life skills of clients

Attend Treatment Team meetings as scheduled.

Participate in client review meetings as needed.

Lead in the development and facilitation of the Individual Service Plans (ISPs) for your client base as well as quarterly reviews (or as needed)

Attend court, including Landlord-Tenant court (as needed if the client does not have appointed legal counsel present)

Enhance the client’s environment by creating a hospitable, customer-oriented environment.

Complete reports and maintain statistical data on an ongoing basis.

2. Conduct a minimum of eight (8) contacts per month (2 per week) with individuals/families, of which two (2) contacts must be face-to-face if the client is unhoused and one (1) must be face-to-face and take place in the home if the client is housed in the PSH Program.

3. Work closely with the Program Manager and Service Coordinators as needed.

Minimum Qualifications:

A bachelor’s degree in social work, psychology, sociology, counseling, or other related social service/science disciplines and a minimum of 2 years’ experience in the social services field.

Certification and licensure in a relevant discipline (e.g., certified addictions counselor, peer recovery specialist) may substitute for education requirements, along with four years of social service experience. In addition, the employee must verify active certification and re-certification throughout employment.

Experience collaborating and partnering with other social service providers.

Ability to think critically and utilize sound judgment.

Ability to maintain client confidentiality and professional boundaries.

Strong knowledge of community resources or ability to become knowledgeable required.

Strong time management and organizational skills are required.

Strong interpersonal and connecting skills are required.

Strong communication skills, including verbal and written.

Proven ability to submit timely, detailed, and accurate case notes.

Proven ability to provide a holistic approach to social services preferred.

Must be able to obtain Police and Child Registry Clearances.

Must have a valid driver’s license and your transportation.

Other duties as assigned.

Working Conditions

This is a full-time position, Monday through Friday, 9:00 a.m. to 5:00 p.m. Occasional evening and weekend work may be required as job responsibilities demand.

Physical Requirements

While performing the duties of this job, the employee will frequently sit, stand, walk, drive, and reach—prolonged periods of driving, walking, and possible meetings.
