DESCRIPTION:

As a member of the Care Management Team, the Care Coordinator is responsible for coordinating the care of low to moderate risk patients. Responsible for accurate and timely tracking of productivity statistics, program status, challenges, updates and developments utilizing data driven methodologies. Provides coordination of care to patients with chronic medical and mental health conditions. Actively promotes and enrolls patients into Care Management Program. Job requires adaptability and ability to work under pressure and meet deadlines; ability to work as a team and contribute to the development of an effective work environment; exceptional communication, organizational and office and interpersonal skills. Performs other duties as needed.

Schedule: 8 hour days, 5 days per week

Location: Barstow, CA

JOB DUTIES AND RESPONSIBILITIES

Performs daily screenings using EMR generated appointment reports and vitals patients.

Alerts provider of need to place order for appropriate screening exam.

Performs care coordination to ensure completion of provider ordered screening exams. Uses relationship-based strategies to engage patients in care.

Ensures that screening results are received timely and entered into the electronic medical record (EMR).

Actively monitors results to ensure appropriate follow up and diagnostic studies are ordered and completed, as appropriate. Assists patients to follow through on their care plan wellness goals, using both phone and in person contact.

Uses established care guidelines to implement provider-directed reminders and recalls in the EMR.

Utilizes EMR generated appointment reports to capture missed appointments. Assists in the coordination of appointment and referrals for physical and behavioral health appointments.

May Performs abstractions of historical screening results into EMR system.

Identifies internal and external challenges related to patient and staff cooperation.

Recommends improvements to processes as appropriate.

Demonstrate the ability to be culturally sensitive and respect diversity, work effectively and with individuals of different cultures and socioeconomic status; Passion for service.

QUALIFICATIONS:

Education/Certification: High School Diploma or equivalent. Graduation from a Certified Medical Assistant Program or CNA certificate program. Medical Assistant or CNA diploma/certificate required. As a requirement of this position you must receive EPIC certification for the module you have been hired into.

Experience: Minimum of three (3) years as a Medical Assistant or CNA in an outpatient or community-based clinic required. Medical knowledge required. Bilingual Spanish/English preferred.

Must have proficient computer skills including data entry (35wpm). Must be able to use a variety of office equipment including fax machine, scanner, printer and copier.

Must have professional telephone etiquette combined with a pleasant and courteous voice. Strong public relations skills are essential. Must possess the ability to maintain a professional attitude in difficult situations and communicate effectively. Must perform as a team player.

Must be legally authorized to work in the United States on a full-time basis. Must not now or in the future require sponsorship for employment visas.
