LVN Hospice

Job Description

We are seeking a dedicated and compassionate Licensed Vocational Nurse to join our hospice care team. The ideal candidate will assist in providing quality end-of-life care and support to our patients and their families. As an LVN in our hospice, you will work closely with our interdisciplinary team to ensure the physical and emotional well-being of our patients during their final stages of life.

**Responsibilities:**

- Administer prescribed medications and treatments in accordance with physician orders.

- Monitor and assess patient health, documenting any changes in condition.

- Provide compassionate and attentive care to patients and their families.

- Collaborate with the interdisciplinary team to develop and implement individualized care plans.

- Assist with pain management and symptom control measures.

- Maintain accurate and up-to-date patient records and charts.

- Educate patients and families about end-of-life care and available support services.

**Qualifications:**

- Current LVN license in CA.

- Minimum of 2 years of experience in nursing, preferably in hospice or palliative care.

- Strong understanding of patient care principles and best practices.

- Excellent communication and interpersonal skills.

- Ability to work collaboratively in a multidisciplinary team.

- Compassion and empathy toward patients and their families.

- Familiarity with hospice regulations and compliance standards.
