Physician - Nephrologist

Physically present during hours of operation of the center and ensure that all patients are discharged stable

before end of his/her duty.

b. Perform initial assessment and medical history upon patient’s admission to the center and documents them at

“Patient History” form.

c. Perform and document pre-HD assessment of patients prior to initiation of HD treatment and post-

HD assessment thereafter at the patient’s Hemodialysis Treatment Sheet.

d. Documents necessary medical findings and doctor’s orders at Doctor’s Orders Sheet.

e. Updates and documents patient’s Problem List as deemed necessary.

f. Updates and documents patient’s maintenance medications at their Medication Sheet quarterly (every January,

April, July, and October) and as necessary.

g. Prescribe medications to patients as necessary.

h. Attends to patients and responsible for the acute care of the patient undergoing dialysis during his/her tour of duty.

i. Reviews and accomplish consents and/or waivers such as “Consent for Hospital Transfer” and “DNR Directive” form.

j. Conducts phone call and/or personal endorsement to the referring hospital for conduction.

k. Coordinates individual patient care with their Attending Nephrologist.

l. Examines staff’s sickness and provide Medical Certificate.

Duly licensed by PRC and:

Board certified in nephrology or,

Board eligible in nephrology or,

Board certified in Internal Medicine (IM), who underwent a course conducted and certified by the PSN or,

Board eligible in IM, who underwent a course conducted and certified by the PSN or,

General practitioner, who underwent a course on POD conducted and certified by the PSN

The certification by the PSN is renewable every three years

Has an updated BLS and (within 3 years) ACLS certification
