TITLE: Case Manager (100 Families)

Fort Smith, AR

Monday-Friday 8 - 5

JOB SUMMARY

Responsible for assessing the immediate needs of clients, connecting them with appropriate resources based on their circumstances, and assisting clients as they move from crisis to career.

Qualifications

To perform this job successfully, an individual must be able to execute each essential duty satisfactorily in a timely manner. The requirements listed below are representative of the knowledge, skill, and/or ability required. This person must be proficient in interpersonal and communication skills, as well as time management and organizational skills. Registration in the state as a Counselor in Training (CIT) or certified as an Alcohol and Drug Counselor is required. Completion of a Qualified Behavioral Health Provider (QBHP) training is required. Work experience with vulnerable individuals and with the criminal justice system is a plus. Must be knowledgeable of all local, state and federal regulatory requirements.

Education and/or Experience

Minimum of a GED or high school diploma; Bachelor’s degree in the human service field is preferred. Applicants must register as a Counselor in Training (CIT) or be certified as an Alcohol and Drug Counselor. Within six months of eligibility to test for the ADC exam, the exam must be scheduled and taken. Must be credentialed as RADD and RDS. The person in this position must be an expert in community resources and have the ability to collaborate with other providers and resources.

Ability to stand, reach, lift, bend, kneel, stoop, climb, push and pull items weighing 25 pounds or less; manual dexterity; auditory and visual skills; ability to follow written and oral instructions and procedures.

Job Duties

Conduct assessments with individuals to assist in determining resources and guide individuals through the resource connection process.

Act as an advocate for clients, serve as a bridge between individuals and providers to increase client involvement in their care plans; provide support to clients at court sessions, DCFS hearings, etc.

Maintain detailed documentation of work with each client, utilizing both Credible and 100 Families case management systems.

Develop and maintain trust and positive relationships with 100 Families partners and

Attend weekly Learning Labs

Participate in 100 Families meetings and

Assist with other residential treatment duties, as time May include drug screening, admission assistance, medication calls, room checks, etc.

If not certified or licensed, must complete an initial -40- hour classroom training to obtain a QBHP certificate. Ongoing training will consist of at least eight hours of training each twelve- month period. The training may, in part, be conducted in the field. Documentation of in-service hours will be maintained in the employee’s personnel files and will be available for inspection by regulatory agencies.

Per insurance requirements, all employees must be at least 21 years of age and have a valid driver’s license.
