We are currently offering $1,000.00 sign on bonuses! Bonuses are payable in two installments ($500.00 with first paycheck, and $500.00 after satisfactory completion of six-month probationary period).

What we’re looking for:

Rock Creek Foundation, an innovative non-profit organization located in Silver Spring, MD, is accepting Part-Time applications for our Contract Delegating Nurse position to support our Residential Rehabilitation Program. This program aims to help individuals with developmental disabilities, mental illnesses, and those experiencing emotional and/or behavioral challenges take charge of their own lives and develop daily living skills needed to thrive independently by providing direct support.

What you’ll do:

Administers hypodermic injections, following Physician's prescriptions and agency procedures

Completes all initial health/nursing assessments for new and current individuals in residential rehabilitation program

Performs all 45 day reviews for all DD individual/s to include but not limited to record review (past and current), takes pulse, temperature, and respiration and other pertinent vital signs, review of medication administration record, PMOF, ensures medical appointments have been completed, etc.

Provides additional Nursing Support hours per identified individual(s) Service Funding Plan for more intense medical diagnoses/conditions

Reviews all doctor orders on a monthly bases

Reviews all monthly cycle medication prior to the 1st of every month and audit completed MMR’s at the end of every month.

Conducts individualized health/medical trainings and/or in-services to meet global needs and/or individual specific

Reviews initial medical forms for new clients, prior to admission to residential program

Takes lead on hospital discharge planning and coordination of care prior to returning to community residential setting for all somatic hospitalizations.

Completes and updates to HRST State-wide system and initiative.

Provides medication review and case management activities for coordination of care with primary physicians, internal/external psychiatrists, insurance companies, and hospitals (including short term hospitalizations)

Ensures safe storage of all medications and medical supplies according to regulations and agency policy at each licensed site

Interfaces with other agency departments to ensure coordination of care

Acts as a consultant and educator to staff, individuals and families regarding health issues, medication and medical conditions

CMT supervision and on-site medication observation

CMT renewal clinical updates, quarterly

Provides medical updates and care plan updates for annual IP’s

Responsible for coordination of care with the RCF treatment team, ASG PRP/Clinic to obtain updated information on our RRP Clients, such as Multi-Axle Assessments and medical updates for our charts

Check pharmacy monthly billings, and ensure charges/claims are appropriate

Review and maintain medical and clinical records kept at office and client sites, to ensure proper filing and train residential staff to maintain them

Completes monthly quality assurance documents for medical requirements

Attends bi-annual IRP meetings and provides medical updates

Liaison with agency appointed Pharmacy

Purging of medical records on a quarterly basis or more often if needed

Responds to medical emergencies and triages accordingly to determine appropriate level of care

Provides telephone support and triage to clinic population as needed

Maintains client caseload and provides treatment coordination according to all applicable regulations and policies

Participates in and successfully completes all required trainings

Consultations with Administrative/Management to ensure agencies compliance with all State and agency regulations, policies and procedures

Travels to St. Mary’s County at least 1x per month

Other duties as assigned

Required Qualifications:

Bachelor’s Degree in a human services or related field of study; OR four (4) years’ related experience and/or training; OR equivalent combination of education and experience.

Must be a Registered Nurse (RN) in the State of Maryland.

Must possess a valid Driver’s License (with no more than two points) and proof of current automobile insurance.

Knowledge of Internet and Microsoft Office software (MS Word, MS PowerPoint, MS Outlook).
