Nurse Practitioner/Physician's Assistant
In search of a Licensed Nurse Practitioner/Physician’s Assistant to work in our outpatient Medication Assisted Treatment Program. This position will task the provider with seeing patients in a medically-assisted recovery program. The Nurse Practitioner/Physician’s Assistant provides Substance Abuse care to the Opioid Use population, including formulating a diagnosis and a treatment plan and providing continued follow-up patient care; prescribing and monitoring medication used in the Opioid Treatment Program.
ESSENTIAL DUTIES
Provide direct primary and acute addiction care services to adult patients
Perform initial and on-going assessment of patients' medical, physical and psychosocial status
Obtain and evaluate health and medical history; perform a physical exam, conduct preventive screening procedures based on age and history
Identify medical and health risks and needs
Complete accurate and timely documentation of observations, interventions, plan of care, referrals and follow-up
Identify needs of the individual and develop a plan of care and/or education plan
Make appropriate referrals to other health professionals and community agencies
Reassess and modify the plan as necessary to achieve medical and health goals
Monitor co-morbid issues and coordinate with primary care and other specialty agency to ensure best practices
Provide medical guidance to the program regarding policy and procedures.
Participate and engage in staffing and care planning meetings regarding patient progress and care.
QUALIFICATIONS:
Licensed Nurse Practitioner or Physician Assistant able to practice in the state of VA
Advanced nursing knowledge and clinical skills through an appropriate educational program
Minimum of 1 year of experience as a practicing nurse practitioner
DEA License
Willingness and ability to get X-Waiver
Ability to work early hours to meet the needs of our clients
Experience in addiction/psychiatric treatment preferred
