Positions:

HHA/CNA Duties

● Meal preparation, light housekeeping, and/or provide companionship and personal care

● Execute on the client’s care plan as outlined by the Care Designer

● Help clients with their ADL's (Activities of Daily Living) that are within the scope of practice for a

● Assist clients with transfers, ambulation and exercises

● Meet the safety needs of the client and use equipment safely and properly

● Function within the limits of your own experience and knowledge, while practicing safely and competently within the job description

● Complete records and documentation accurately and thoroughly

Requirements for position:

● Has completed a HHA/CNA program and be licensed with the state of Indiana (or registered to test for certification);

● Negative TB skin test or chest x ray (less than 2 years old)

● 2 Professional references

LPN/RN

Duties

● Skilled and non-skilled assessments, Administer medications, provide treatment, health promotion/education, IV Infusion therapy

● Personal care, meal preparation, light housekeeping, and provide companionship

● Help clients with their ADL's,assist with transfers, ambulation and exercises.

● Assist client to do what they love to do

● Must be able to speak and write in English. Other languages will be an asset.

● Ability to identify and report client care concerns when needed.

● Able to travel from client to client in a timely fashion. Drivers with own vehicle

Requirements for position:

● Active LPN/RN Licence in the state of INDIANA with no restrictions to practice

● Current CPR Certificate

● Negative TB Test Results

● Excellent verbal and written communication

● Ability to work reliably and independently

Job Types: Full-time, Part-time, PRN

Salary: $15.00 - $50.00 per hour

High school or equivalent (Required)
