LPN Case Management Supervisor

Job Description

We are seeking an LPN Home Health to join our team! You will be responsible for delivering high quality care to assigned patients.

The LPN manageer will perform comprehensive assessments to evaluate the patient’s health care needs and social determinants of health to develop individualized and person-centered care plans, and monitor and re-evaluate the plan as necessary. Monitor MARs monthly, and medication management .Also, identify patients with recent emergency department visits or hospital admission, and assist them with transitioning back to their primary care provider including medication reconciliation and close gaps in care.

Responsibilities:

Conducting onsite surveys to evaluate the performance and effectiveness of care providers

Provide clinical nursing perspective in the assessment of policies, projects, and data related to the measurement of quality, legislative and administrative proposals, and make recommendations to owner

Deliver clinical practice advice as part of the DBHDD program policy or support teams responsible for interpreting applicable laws, regulations and policies regarding highly complex issues in MEDICAID administered programs

Coordinating medical treatment, including administering assessments, developing care plans, monitoring medication compliance, and more

Supervising Direct Support Staff

Serving as advocates for and champions of the patients' health

Connecting patients with social and community support systems for the continuation of care

Making sure labs, physician appointments, and all medical appointments are met quarterly.

Collecting the data for bi-monthly Nursing assessments

Qualifications:

Previous experience in nursing or other medical fields

Familiarity with medical tools and equipment- Excel- EMR Applications

Ability to build rapport with patients

Compassionate and caring demeanor

Excellent written and verbal communication skills

Having some experience in the intellectual development, disability communityCompany Description
