Social Worker III

Your Opportunity: This Advanced Practice Position provides counselling and support to clients within the palliative care program in the Medicine Hat and Brooks area.

The SW III provides bio-psychosocial assessment, resource navigation, crisis intervention, education, supportive counselling, advocacy and consultation as part of their day-to-day work.

They will have excellent working knowledge of community financial resources and medical coverage for adults and families.

They will make appropriate referrals to services that are both internal and external to the healthcare setting and works collaboratively with community agencies/programs for the benefit of clients and families.

Description: As a Social Worker III, you will perform either advanced level assessment and treatment or act as a clinical lead.

You will work with or within teams to provide health care services for patients with complex and multidimensional medical, psychosocial, environmental and instrumental needs.

You will provide advanced clinical services, consultation to the interprofessional team and patients/families and supervision of students and team members as assigned.

You may also act as a clinical lead providing leadership in the planning, development, and delivery of services in a specific area, as well as staff development, quality improvement and evaluation or research activities as identified by the needs of the service area(s). Active or eligible for registration with the Alberta College of Social Workers (ACSW). Additional Required Qualifications:Demonstrate 3-5 years' experience in a health care setting, preferably working with individuals and families in the end stage of life.

Demonstrate ability to navigate within a complex medical system and function within a fast-paced multidisciplinary environment.

Working knowledge of programs and systems including Microsoft Office Suite, Meditech, Connect Care, Netcare, etc.

Preferred Qualifications:Excellent verbal and written communication skills.

Knowledge of relevant legislative Acts and experience with guardianship, trusteeship, personal directives and capacity assessment processes is an asset.

In-depth knowledge and in working with Income Support programs at a community/provincial/federal level (AISH, Alberta Works, CPP, OAS, ASB) is preferred.

Comfortable working with clients and families of all ages and in various stages of grief and loss.
