Position: Home Health CNA

Job Summary: To provide routine nursing care in accordance with established policies and procedures as directed by supervisory personnel

Essential Functions and Responsibilities:

Provide nursing care to residents/clients within the scope of practice permitted by certification, performing tasks in a sensitive, caring manner.

Follow the Home Health Aide Care Plan in each setting as established by the registered nurse

Receive assignment and resident report at the beginning of the shift. Make rounds to assure residents are clean, comfortable and not in any distress. Report any adverse findings to the nurse immediately.

Complete necessary forms and documentation in a timely manner. Report responses, needs or findings to the nurse for appropriate intervention.

Assist residents/clients with nutritional intake. Encourage and support residents to function at the highest level of independence, providing assistance or use of supportive devices when needed. Maintain a constant supply of fresh water at the bedside and offer extra nourishment if allowed. Document and report intake and output as directed.

Routinely provide assistance to residents which requires, lifting, pushing and balancing the equivalent of 50 lbs.

Provide assistance with personal hygiene, bathing, grooming, oral care and general cleanliness. Encourage self-help measures when warranted.

Assure that residents/clients are dressed appropriately and that clothing is neat, clean and presentable. Residents/clients should have stockings/socks and comfortable, well fitting shoes. Promote self help measures, offering assistance as needed.

Provide for and assist with elimination needs. Use gloves to avoid contact with body fluids. Use incontinence products when directed, checking residents at least every two hours, and providing care as needed. Assist with restorative program to encourage resident to function at their highest level.

Keep environment clean and safe. Keep hallways clear and floor free of debris. Follow Guidelines for Infection Control. Clean equipment and/or work areas assigned.

Assist with admissions, transfers and discharges as requested. Assure that residents’ clothing personal articles, equipment and supplies are properly stored and labeled according to policies and regulations.

Encourage residents to participate in activities and social functions. Assist with transportation when needed.

Accompany residents/clients on appointments/outings.

Cooperate with other staff and departments to provide quality services to residents/clients

Complete assignments in allotted period of time.

Other duties as requested or assigned by supervisor or scheduler.

Working Conditions:

Travel required during all seasons to assigned patient homes and work in various home conditions

May be exposed to noises or sound levels that are distracting. Medical equipment can sometimes be loud

Work very near others. Routinely comes in physical contact with patients throughout your shift

Often exposed to contaminants

Often exposed to diseases or infections from contact with patients

Sometimes wear safety gear, such as gloves, face masks, and back support

May have to get into awkward positions to reach cramped work spaces

Educational, Physical, and Other Requirements:

Employee must be able to perform the essential functions of the job with or without reasonable accommodations. Physical requirements may include frequent use of hands to finger, handle, or feel objects, tools or controls; reach with hands and arms; talk or hear. The employee may be required to lift, stand; walk; sit; climb or balance; stoop; kneel; crouch or crawl; and smell. Vision abilities may require close vision, distant vision, color vision, peripheral vision, depth perception and the ability to adjust focus.

Provide assistance to residents which may require lifting, pushing or balancing 50 pounds or greater

For the safety and security of our residents and patients, employees must be able to read, write, and verbally communicate in English, which is the most universally used language of our resident and employee population

Must be a Certified Nursing Assistant in good standing with the state of Florida

Pass a written & practical competency

Meet the CNA continuing education requirements and attend required in-services and training

Current CPR certification

Adherence to agency safety and infection control practices

Must be flexible and capable of maintaining regular attendance

Must meet all local health regulations and pass a pre employment and subsequent routine physical exam, drug screen and background check

Must maintain a professional, respectful, friendly demeanor with all residents, family members, guests, and staff at all times.
