Position Summary: The Psychiatric Mental Health Nurse Practitioner is responsible for providing direct psychiatric care needs (Acute and Chronic Care) to individuals of the Crossroads 360 Community Care Clinic.

SCOPE OF PRACTICE: Assessment of the psychiatric needs of the client and implementation of a plan of care through diagnostic, preventative, and therapeutic services in accordance with all state, federal, and licensing standards and guidelines.

MINIMUM QUALIFICATIONS:

Current State of Arizona RN license and BLS certification.

2. Current license to practice as a Nurse Practitioner and state authority for advanced practice.

Possession of prescription and dispensing privileges required.

Board Certification.

Current and Valid State of Arizona Driver’s License Required.

DESIRED QUALIFICATIONS:

Doctor of Nursing Practice degree or PhD. Preferred.

MAT waivered.

CONTINUING EDUCATION:

Per licensing and certification requirements.

Essential Duties and Responsibilities:

Duties/Responsibilities:

Diagnoses psychiatric disorders and mental health conditions. Distinguishes between physiologically and psychologically based disorders and makes appropriate diagnoses.

Assesses patients’ mental and physical status based on the presenting symptoms and complaints. Conducts individual, group, and family psychotherapy for those with chronic or acute mental disorders.

Documents patients’ medical and psychological histories, physical assessment results, diagnoses, treatment plans, prescriptions, and outcomes. Writes prescriptions for psychotropic medications as allowed by state regulations and collaborative practice agreements. Monitors patients’ medication usage and results.

Evaluates patients’ behavior to formulate diagnoses or assess treatments.

Educates patients and family members about mental health and medical conditions, preventive health measures, medications, and treatment plans.

Collaborates with interdisciplinary team members including psychiatrists, psychologists, and nursing staff, to develop, implement, and evaluate treatment plans.

Required Skills/Abilities:

Ability to diagnose and treat simple and complex psychiatric and mental health problems from adjustment disorders to serious mental illnesses such as schizophrenia, bipolar disorder, major depression, and anxiety disorders.

Strong skills to manage, coordinate, and evaluate the care and education of the patient and family along the health care process.

Ability to manage patients who need to be closely monitored.

Trained to adhere to medications and therapies with complex protocols.

Expertise in handling sophisticated medical equipment.

May be required to work nights, weekends, and holidays.

On-call duty may be required (available to work on short notice).

Administrative

Maintain accurate medical records using Next Gen EMR and proper forms in a timely manner. Supervise and train clinical staff and volunteers on data entry into the electronic health records system and provide oversight that original data is entered correctly.

Track client data that will demonstrate measurable success of Clinic services. Generate and analyze reports to document outcome measures from the electronic records. Must be proficient in developing & analyzing outcome measures.

Supervise and precept student volunteers.

Consistently use supplies and equipment in a cost-effective manner.

Recommend needed supplies and equipment for delivery of care.

Quality Assurance

Participate in quality improvement activities as directed by Executive Director and/or Chief Medical Officer in coordination with the Executive Director.

Provide education to clinical staff as needed.

Communicate effectively with clients and their families in a respectful manner.

Maintain an acceptable caseload to meet productivity needs.

Utilize updated written protocols as mandated by the Arizona Board of Nursing Statutes and Regulations.

Ensure that care provided is consistent with community standards of care within the limitations of the program.

Follow established safety precautions in performance of duties to ensure a safe environment.

Support policies and philosophy in delivering health care as set forth by Crossroads 360 Community Care Clinic and Crossroads Board of Directors.

Other Duties: Other duties as assigned. Performs other duties as identified by the Chief Clinical Quality Officer to assist in clinic operations and staff supervision. If practicing under the faculty practice model, other duties outlined via the faculty practice model.

Qualifications:

Valid, state-issued Nurse Practitioner license in good standing.

Certified by the Specialties Board in his/her respective specialty.

2+ years’ clinical experience in a clinical care setting.

Experience and demonstrated successful leadership of case management, disease management and/or population health programs.

Experience with clinical risk management and safety protocols.

Willingness to adhere to all in-house and legal regulations.

Familiarity with EHR and EMR records systems.

Excellent leadership, communication, problem solving and organizational skills.

Current CPR/First Aid training.

Ability to work independently and as a member of a team.
