Hospice of the Piedmont is seeking applicants for a full-time Registered Nurse to serve in our Center for Acute Hospice Care (CAHC) for 24 hours per week AND Hospice House (HH) for 12 hours per week. Under the direction of the Clinical Services Manager, the RN will plan, organize, and direct inpatient hospice care. The ideal candidate will identify patient/family/caregiver needs and provide nursing and supportive care in accordance with the plan of care and organizational policies and procedures. This position is for night shift.

Responsibilities/General Duties:

Patient Care:

Complete initial assessments of patients and families to determine hospice needs.

Provide professional nursing care utilizing all elements of the nursing process.

Assess and evaluate patient status, regularly re-evaluating needs and revising the plan of care as necessary.

Initiate appropriate preventive and restorative nursing procedures, including administering medications and treatments as prescribed.

Counsel patients and families in meeting nursing and related needs.

Provide health care instructions to patients as appropriate and assist with activities of daily living.

Report changes in patient condition to the IPU Clinical Leader and communicate with attending physicians or Hospice Medical Director.

Document appropriately in EMR and assist in maintaining health, fire, and safety measures.

Ensure utilization of universal precautions for infection control at all times.

Assist in conducting clinical observations of CNAs and LPNs and prepare written evaluations.

Perform narcotic count with shift change and adhere to policies/procedures for safekeeping and destruction of medications.

Provide on-call and holiday coverage as assigned.

Communication:

Maintain accurate and relevant clinical notes regarding patient condition and care given.

Communicate with physicians regarding patient needs and report changes in condition; obtain/receive physician orders as required.

Teach patients/families regarding end-of-life care and provide medication, diet, and other instructions as ordered.

Work in concert with other Hospice Interdisciplinary Team Members.

Provide and maintain a safe environment for patients and families.

Assist in providing continuity of care and meeting emotional needs of patients and families.

Participate in Interdisciplinary Group meetings as requested/assigned.

Additional Duties:

Ensure arrangements for equipment and other necessary items/services are available.

Direct/supervise inpatient unit CNAs and delegate responsibilities when required.

Assume responsibility for personal growth and development through continuing education and in-service classes.

Actively participate in quality improvement programs.

Fulfill requested and/or accepted assignments.Requirements:

Qualifications:

Graduate of an accredited nursing school.

Licensed as a Registered Nurse (RN) in the Commonwealth of Virginia.

Minimum of 2 years hospice, home health experience, ICU CCU experience.

CHPN preferred.

Knowledge, Skills, and Abilities:

Basic knowledge of Medicare/Medicaid reimbursement regulations.

Current CPR Certification.

Emotionally mature and caring individual willing to provide support and assistance to terminally ill and their families.

Ability to effectively present information and write reports.

Good oral and written communication skills.

Skills in establishing and maintaining effective working relationships.

Ability to manage multiple tasks and work as a team member.

Flexible and able to function in stressful situations.
