Position Description

Overview

The Customer Service Professional reports to the Director of Care Coordination with accountability for providing strategy, judgment, organization, and evidenced-based analysis to influence decisions, and directly to meet CINQCARE requirements. At CINQCARE, we don’t have patients or customers – we have Family Members.

Job Responsibilities

The Customer Service Professional will have the following responsibilities:

The Customer Service Professional is responsible for providing compassionate customer service and issue resolution to our Family Members.

This role is responsible for coordination of CINQCARE’s Family Member navigation plans via our PCP providers and ACO members.

The ideal candidate will support clinical and non-clinical staff supporting Family Members enrolled in UFP & CINQCARE

The Customer Service Professional will be responsible for scheduling, cancelling, and rescheduling appointments for our Family Members.

The Customer Service Professional is responsible for verifying Family Member’s medical insurance eligibility.

Ensures proper authorization or referral is obtained from referring physicians, insurance, and/or CINQCARE’s Family Members.

Facilitate CINQCARE’s Family Members flow by efficient checkout of Family Members - schedule follow up appointments, testing, and referrals.

Follow up with CINQCARE’s Family Members to confirm upcoming appointments and verify demographic information required at the time of the appointment.

Performs other job-related duties as assigned.

Qualifications

The Customer Service Professional should have the following qualifications:

Education: High School Diploma, HSE, GED.

Experience: At least 1-2 years of call center experience. Medent EMR experience is a plus; but not required. Proficiency in all Microsoft Office applications.

Entrepreneurial. CINQCARE seeks to fix gaps that have persisted for generations in the delivery of care to Black and Brown populations. This position is accountable for ensuring CINQCARE is positioned to innovatively deliver on its promise.

Communication: Bilingual Spanish and English preferred. Excellent verbal, written communication, and presentation skills; ability to clearly articulate and present concepts and models in an accessible manner to CINQCARE’s team and community partners.

Relationships: Ability to build and effectively manage relationships with our Family Members and their team.

Location: Buffalo

Compensation: $16- $16.92
