Industry: Medical Type: Contract (Duration not determined) Start Date: ASAP (Immediate) Position Overview: The Case Manager will play a pivotal role in coordinating and managing patient care from admission to discharge, ensuring continuity, quality, and efficiency throughout the healthcare journey.

Working closely with interdisciplinary teams, the Case Manager will assess patient needs, develop individualized care plans, and facilitate communication and collaboration among healthcare providers, patients, and families.

Key Responsibilities: Assess patient needs, preferences, and goals to develop comprehensive care plans in collaboration with healthcare providers.

Coordinate and facilitate the delivery of patient care services, including medical treatment, therapies, and support services.

Monitor and evaluate patient progress, outcomes, and satisfaction with care plans, adjusting as necessary to ensure optimal outcomes.

Communicate and collaborate effectively with healthcare providers, patients, families, and external stakeholders to facilitate care transitions and continuity.

Advocate for patients' rights, preferences, and needs throughout the healthcare journey.

Document patient assessments, care plans, interventions, and outcomes accurately and timely.

Participate in interdisciplinary meetings, case conferences, and quality improvement initiatives as required.

Minimum Requirements: Registered Nurse qualification with current registration with the South African Nursing Council (SANC). Minimum of 3 years of experience in case management within a private hospital setting.

Strong understanding of clinical processes, patient care pathways, and healthcare delivery systems.

Excellent assessment, planning, and organizational skills.

Effective communication and interpersonal skills, with the ability to collaborate with diverse stakeholders.

Ability to work independently and as part of a multidisciplinary team in a fast-paced environment.

Proficiency in electronic medical record systems and Microsoft Office applications.
