Responsible for acting as an information resource to patients, physicians, nurses, and other healthcare professionals regarding medication therapy and drug related information, and may serve as a clinical liaison for the healthcare team for the Medication Management program. The goal of this program is to optimize therapeutic outcomes by focusing on safety, effectiveness, lower-cost alternatives and adherence. This pharmacist will utilize their clinical judgement and expertise to ensure the best medication outcomes for their patients. The pharmacist will support the dispensing pharmacies by helping manage queues, proactively managing refills, balancing workloads, triaging phone calls, and providing billing support. Responsible for the filling and dispensing of prescriptions including the proper handling, usage, and storage of drugs in the pharmacy according to state and federal laws. Maintains standards set by the department and translates policies and procedures into the daily operation of the pharmacy. Responds to inquiries concerning drug contents, dosages, regimen reviews, errors, storage, and disposal. Monitors drug therapies for drug interactions, allergies, and contraindications. Monitors stocks of drugs and supplies. Inventories and procures drugs, medications, and supplies as required. May oversee support staff and assist in training new staff.
Daily tasks will be completed independently and prioritized based on the patient and pharmacist needs, maintaining patient confidentiality and HIPAA at all times.
Salary Range:
$125,000-$135,000 Annually
The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Job Requirements
Education: Meets the educational qualifications to be a licensed Pharmacist in NYS. Minimum BS degree in pharmacy from an accredited college of pharmacy
Experience: Preferred
License/Certification Required: Current NYS license (or eligible for licensure in NYS)
