Licensed Practical Nurse

LPN - Specialized Care

Location: Springfield, MA

Schedule: Monday to Thursday, 7:00 AM to 3:00 PM (with flexibility)

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team near Springfield, MA. As an LPN, you will play a crucial role in providing care to patients with developmental disabilities, brain injury, and mental health disabilities.

Responsibilities:

Conduct nursing assessments and develop individualized care plans.

Provide Direct Care to patients in program.

Monitor patient progress and collaborate with the interdisciplinary team.

Provide support and education to patients and their families.

Qualifications:

Current LPN licensure in Massachusetts.

Experience in caring for individuals with developmental disabilities, brain injury, or mental health disabilities is a plus.

Strong communication and interpersonal skills.

Experience with G-Tubes

Experience with total care.

Flexibility in schedule and shifts.
