Overview of the Position:

The Advance Practice (APP) will is responsible for the direct care of patients in the primary care setting. The patients include pediatric, adult and geriatric patients care provided through agreed upon protocols. The Advance Practice Provider will practice with preceptorship provided by a licensed Physician.

Education and Experience:

• Graduate from an accredited Physician Assistant program. This may be from a college or other accredited organization. Pass the National Certifying Examination and licensure in the State of South Carolina as a Physician Assistant or Master of Science Degree in Nursing from a state approved, accredited training program. Current Nurse Practitioner licensure and maintains official recognition as Nurse Practitioner by South Carolina State Board of Nursing.

• Maintains National certification and eligibility for prescription authority in South Carolina.

• Minimum of at least two-years clinical experience as a Physician Assistant or Nurse Practitioner

• Work requires the ability to exchange information on factual matters, communicate in a variety of manners with patients, staff, and explain policies of the Community Health Center.

• Ability to apply sound medical judgement to carry out instructions provided in written, oral or diagram form.

• Ability to solve problems involving several concrete variables in standardized situations.

Prior to performing job related activities in this position one must understand company and program policies and procedures as well as be aware of HopeHealth’s mission, vision and values. Ability to communicate effectively utilizing both oral and written means.

Ability to take initiative and handle various tasks simultaneously while working efficiently, effectively, and Knowledge of adult learning principles, management leadership principles, and community resources.

Knowledge of policies, standards, regulations, and protocols.

Essential Job Functions:

These functions will be performed within the clinical setting and will be the responsibility of the incumbent working with HopeHealth’ s patients, leadership, administrative team, clinical and non-clinical staff members to ensure excellence in service.

• Provides primary care to patients within approved written protocols

• Works collaboratively with preceptors to provide care to patients whose health status is beyond the scope of a Nurse Practitioner or Physician Assistant protocols

• Refers to other providers when deemed appropriate

• Provides patient education

• Provides counseling services as necessary and refers to appropriate agencies

• Participates in staff education on both a formal and informal basis

• Participate in on-call / afterhours rotation

• Other duties / responsibilities as needed for departmental efficiency

Physical Requirements:

Hearing: Adequate to perform job duties in person and over the telephone. Speaking: Must be able to communicate clearly to patients in person and over the telephone. Vision: Visual acuity adequately to perform job duties, including reading information from printed sources and computer screens. Other: Requires occasional lifting and carrying items weighing up to 10 pounds unassisted in normal daily activities and up to 30 lbs. for other required work activities. Additionally, requires frequent bending, reaching, and repetitive hand movements(especially if keyboarding and writing), standing, walking, squatting, and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular work shift.
