Are you passionate about helping individuals with mental illness achieve greater independence and social integration?

As an Intensive Transition Specialist in our Community Readiness Support Program (CRSP) based in Fairfax County, you'll play a vital role in empowering clients to develop essential life skills and navigate their recovery journey.

What you'll do: Manage client caseloads and develop personalized recovery plans focused on individual goals and empowerment.

Design and deliver culturally relevant psycho-educational groups to enhance clients' health and wellness, independent living skills, and interpersonal skills.

Provide individual and group counseling services, including dual diagnosis support for co-occurring substance use disorders.

Develop and implement coping skills training, helping clients build healthy behaviors and manage their recovery.

Intervene in crisis situations, conduct safety assessments, and refer clients for emergency evaluations when necessary.

Coordinate care with case managers, healthcare providers, and other professionals to ensure holistic support.

Advocate for clients' needs and ensure access to essential services and resources.

Maintain meticulous clinical documentation in accordance with PRS policies, licensing regulations, Medicaid, and CARF standards.

Identify and screen for additional support needs, such as substance use treatment, medication management, and physical healthcare.

Collaborate with the team to continuously improve service delivery and client outcomes.

Participate in professional development opportunities to stay current in best practices.

What we're looking for: Bachelor's degree in Social Work, Psychology, or a related human services field, with experience working with adults with mental health disorders.

Qualified Mental Health Trainee (QMHP-T) with a path to licensure; Qualified Mental Health Professional - Adults (QMHP-A) preferred.

Strong written and verbal communication skills, with a talent for clear and engaging presentations.

Self-motivated, highly organized, and adept at managing time effectively.

Ability to thrive in a collaborative team environment while also working independently.

Proven ability to build rapport with clients, staff, and external providers.

Experience with electronic health records (EHR) systems, preferably Credible EHR.

Strong computer skills and a willingness to learn new technologies.

Deep empathy and understanding of adult mental health challenges.

Must be fully vaccinated to meet contractual requirements.
