RN Hospice Lead Nurse

$10,000 Sign on Bonus

Facility based opportunity with a 5 Star Facility

What will I do in this role?

The RN Hospice Lead Nurse is responsible for ensuring that hospice patient care is coordinated and managed appropriately. The role includes identification of residents for hospice, education, supporting transition to hospice and coordinating hospice care in collaboration with residents, families, staff and medical personnel. Delivers hospice intake referral services, assessment and care management to hospice patients. Develops & promotes hospice programs and services while maintaining the standards of practice consistent with quality hospice care. Works collaboratively with others to provide coordinated care.

As an RN Hospice Lead Nurse, supporting our mission will include the following responsibilities:

Maintains a working knowledge of current Hospice Conditions of Participation, coverage guidelines and state regulatory requirements: admission criteria, eligibility and documentation requirements.

Complies with accepted professional standards and principles.

Completes intake, referral information, assessment and care plan for hospice patients and families. Deliver hospice service in accordance with Medicare regulations and mandates and maintains the comfort and quality of life for those who are diagnosed with terminal diseases.

Case manages all hospice patients at all three Judson campuses.

Develops aide care plan and supervises hospice aide q 14 days.

Fosters positive work environment and culture that promotes unity and collaboration between skilled, AL, IL staff and hospice.

Effective communication with families. Actively listens to family concerns and identifies solutions.

Prepares for and participates in biweekly IDG meetings to facilitate care coordination.

Promotes and participates in Quality Improvement initiatives and QAPI for hospice.

Assists with Hospice new employee orientation and training.

What do I need for this role?

Current Ohio Registered Nurse (RN) License required.

At least 2 years of Hospice experience required.

Ability to routinely travel locally and periodically work a non-traditional schedule/outside regular business hours if needed.
