Minimum Qualifications/Requirements: 1-year experience with a valid license with their appropriate nurse position. Must have valid CPR/First Aid (BCLS) certification, preferably Health Care Provider certification through American Heart Association. Must have knowledge of 12 step programs, one year sobriety if recovering. Knowledge in detox of alcohol and drug clients. Must know medications and side effects. Knowledge with doctor transcribing, telephone orders, nurse charting, assessments, admissions and discharges. Suggested to have IV/ blood draw training and completion certification. Must communicate with client(s), physician (s) and therapist (s) on a professional and therapeutic level. Must complete training and a competency exam prior to commencing employment.

Job Duties:

· Receive verbal report upon start of shift from the nurse you are relieving. Read communication log and sign that you have read it. Perform medication count for controlled/locked medications with witness, such as another Nurse or Technician.

· Round on all clients; assess their vitals and monitor their withdrawal symptoms in accordance with their current level of care (ie Detox or Residential). Detox clients should be rounded on every 1-2 hours or per doctor request. Residential clients should be rounded on as necessary for monitoring of vitals, unless otherwise specified in doctors’ orders. Contact the doctor as necessary for any changes in level of care. Prepare nursing notes for any assessments and care given.

· Review client charts for any new doctor orders, new medications, and related transcription. Review MAR folder for medications to be given on your shift.

· Complete inventory for client medications and send orders out to pharmacy. Follow-up with the pharmacy as necessary to ensure medications are delivered timely.

· Check to see if any clients are going on pass/appointment during your shift and make sure pass/appointments sheets have been completed, with arranged transportation as necessary. Ensure the necessary documentation has been prepared for the appointments/pass.

· Responsible for admissions, discharges, and transfers during your shifts with assistance from technician on duty. Conduct admit to facility for new clients, including proper completion of our documentation in accordance with our policies and procedures, search of items brought to facility and lock-up of contraband and medications.

· Obtain consents from clients; request documentation from previous facilities as appropriate.

· Observe and monitor client behaviors and intervene based on schedule, individual client’s treatment plan and facility needs.

· Assess when clients are harmful to self or others and follow the emergency team protocol as necessary

· Oversee any AMA and ATA discharges and contact the necessary administrators, therapist, and/or doctors.

· Check in with clients weekly concerning their progress, medical needs, psychiatric needs, and follow-ups. Document in client charts.

· Assist with the facilitation of client’s daily activities in accordance with their current level of care.

· Attend and participate in monthly staff meetings

· Assist technician to help ensure physical safety and security by conducting regular facility rounds per policy and procedure.

· Orient new clients to the facility, including expectations and rules.

· Other duties as described in the Nursing Checklist and/or as assigned by the Head Nurse, Executive Management or Facility Administrators.
