The Primary Care Co-located Therapist is responsible for providing assessment, treatment, and coordination of care for patients and families within one or more of our Nationwide Children’s Close to Home Primary Care Centers. This individual will work as part of a multidisciplinary team alongside other professionals on the Primary Care team: psychologists, physicians, nursing staff, pharmacists, administrative staff, and clinical social workers. The Co-Located Therapist offers brief, evidence based therapy for patients ages 0-22 and their families to address a wide range of presenting behavioral health concerns. The role offers opportunities for integrated consultation during patients’ medical visits, as well as education and collaboration with medical staff to ensure comprehensive, equitable and effective treatment of behavioral health concerns. Ideal candidates will be passionate about helping communities and population health as well as have experience working with diverse cultural and economic populations.

Responsibilities

Remains current in required licensure and clinical or other training as required.

Accurately assesses and documents client presenting problems, history, symptomology, mental status, diagnosis, and prescribes initial interventions in a timely manner.

Meets with client/family to assist in developing specific, realistic, and measurable treatment goals and objectives. Employs strategies of intervention that are consistent with empirically supported treatments and consistent with program expectations.

Remains current in general knowledge of psychopharmacology in pediatric behavioral healthcare and collaborates consistently with team medical providers.

Completes clinical charting and other related administrative paperwork in accordance with policy standards.

Practices within professional areas of expertise.

Maintains productivity expectations.

Effectively coordinates client care with other Primary Care team members, service providers and community resources to assure continuity of care.

Meets regularly with Clinical Coordinator.

Attends routine staff meetings.

Attends in-service trainings, peer review and other intra-agency and supervisory activities.

Demonstrates willingness to seek/accept direct supervision as well as respect productive feedback from colleagues and others.

Strives to maintain positive patient satisfaction.

Supports the philosophy, goals and mission of Nationwide Children’s Hospital.

Performs other duties as assigned by Clinical Coordinator or Administrator.

Qualifications

KNOWLEDGE, SKILLS AND ABILITIES REQUIRED

Masters or doctoral degree from an accredited university or professional school of Counseling, Social Work or Marriage and Family Therapy

Appropriate Ohio licensure

Demonstrated skills and abilities in providing and managing behavioral healthcare to children, adolescences, and families

Excellent communication and interpersonal skills in order to establish and maintain positive relationships with patients, families, peers and community representatives
