Position Summary:

Clinical Hospital Pharmacist by using established policies and procedures is responsible for:

· Drug therapy of assigned patients

· Provision of clinical drug information to meet the needs of the physician, nurse, and patient, education of pharmacy students, residents, and interns

· Distribution of medications.

Duties & Responsibilities:

Evaluate medication therapy for appropriateness considering patient conditions, safety, efficacy, formulary status, and interactions.

Provide therapeutic drug level monitoring and communicate with healthcare providers for therapy optimization.

Identify trends, collect data, and formulate recommendations for practice improvement.

Present findings to relevant committees and receive exceptional feedback from medical staff.

Demonstrate accuracy, reliability, and efficiency in dispensing and labeling tasks.

Complete tasks promptly and meet deadlines consistently.

Communicate effectively with supervisors and colleagues, suggesting improvements in procedures.

Provide coaching sessions, contribute to training programs, and engage in professional development.

Publish or present research findings regularly and participate in multidisciplinary committees.

Exhibit proficiency in area-specific technology, suggesting enhancements, and solving technical problems effectively.

Lead or participate in Drug Utilization Evaluation (DUE) or Target Drug Program (TDP) activities.

Comply with documentation requirements, supervise supportive personnel, and demonstrate behaviors aligned with organizational values.

Experience:

PGY 2 in Oncology/Hematology or related experience.

Education:

Pharm.D. degree and residency training or equivalent proven experience is required.

License Certification:

Possession of or eligible for Florida pharmacist license is required.
