Position Title: Case Manager & Senior Case Manager

Reports to: Shelter Director

Supervises: Assigned Case Managers (Senior Case Manager only)

Locations: All New Hope Housing Locations

Position Overview: New Hope Housing is actively seeking qualified candidates for Case Manager and Senior Case Manager roles. These vital positions involve managing and delivering comprehensive services to residents grappling with social, economic, health, and behavioral challenges. The roles require proactive engagement with our service population, assessing and identifying client needs, and facilitating access to appropriate services. Case Managers play an instrumental role in planning and implementing tailored interventions to promote stability and well-being among our clients, aiming to effectively address and ameliorate the challenges of homelessness within the community.

Case Manager:

Case Coordination:

Implement case management programs designed by senior staff.

Manage day-to-day individual cases.

Participate in intake processes.

Service Delivery:

Deliver direct services to guests.

Collaborate with community organizations for service facilitation.

Housing Planning:

Assist in housing planning under senior guidance.

Make referrals to housing resources.

Senior Case Manager:

Program Oversight:

Ensure smooth operation of case management services.

Oversee program implementation including intake and coordination.

Leadership and Strategy:

Lead the case management team and housing initiatives.

Develop and implement strategic case management programs.

Community and Organizational Collaboration:

Collaborate with community organizations and housing managers.

Provide consultation and expert advice.

Supervision and Development:

Mentor and lead case managers and junior staff.
