FUNCTION

This position is responsible for providing prevention navigation services and direct substance abuse counseling and therapeutic services to AFC clients, with a focus on our TGNC clients.

TOP RESPONSIBILITIES

●Facilitate client care with responsibilities including appointment scheduling, client introductions during clinic appointments, health care navigation and escorts, insurance assistance, community resources and referrals, and care coordination for TGNC clients.

●Provide direct support to partner clinical providers regarding hormone replacement therapy needs, surgical referrals, resource guidance, and client care follow-up.

●Maintain a caseload of ongoing clients focusing on substance use issues, and facilitate therapeutic and activity groups focused on substance abuse.

●Attend weekly treatment planning meeting, department staff meetings, and individual supervision.

MSW required or CASAC

Salary: $64,800

TECHNICAL SKILL REQUIREMENTS

Proficiency in Microsoft Office programs and ability to navigate database software.

QUALIFICATION NEEDED

The successful candidate for this position will have relevant street outreach experience, demonstrated leadership skills, and knowledge of LGBTQ youth populations. General knowledge of HIV/AIDS and other sexually transmitted infections and harm reduction strategies is also required. Bilingual speaker preferred. Must be able to work night and weekend hours.
