Registered Nurse - Hospice

Position Overview:

We are seeking a skilled and empathetic Registered Nurse (RN) to join our hospice care team. The ideal candidate will be passionate about delivering high-quality care to patients in their homes or hospice facilities. As an RN Hospice Nurse, you will work closely with patients, families, and a multidisciplinary team to ensure comfort, manage symptoms, and provide emotional support during end-of-life care.

Responsibilities:

Conduct initial assessments and develop individualized care plans in collaboration with the interdisciplinary team.

Administer medications, manage symptoms, and provide skilled nursing care to maintain patient comfort.

Educate patients and families on disease progression, treatment options, and end-of-life care.

Regularly assess patient needs and coordinate care with physicians, social workers, chaplains, and other healthcare professionals.

Provide emotional support and guidance to patients and families through compassionate communication and active listening.

Maintain accurate and timely documentation of patient care and progress.

Qualifications:

Active Registered Nurse (RN) license

Minimum 2-4 years of nursing experience, preferably in hospice, palliative care, or a related field.

Strong clinical skills with a focus on pain and symptom management.

Excellent communication and interpersonal skills.

Compassionate and empathetic approach towards patients and families facing end-of-life care.

Ability to work effectively in a multidisciplinary team.
