Overview of Position: Assist with the completion of tasks and projects in service to and in conjunction with the Clinical Pharmacy Team.

Qualifications: Enrollment in an accredited Doctor of Pharmacy program.

Knowledge/Skills: Excellent public speaking, presentation, written communication skills, and computer skills. Strong interpersonal/interprofessional skills and ability to multitask.

Essential Job Functions:

• Gather patient records and collect pertinent medical record information

• Appropriately prepare and/or administer vaccinations

• Provide patient counseling including, but not limited to medication information, administration, adverse effects, and monitoring

• Perform chronic care management (CCM) services

• Make recommendations for therapy modifications as appropriate and as agreed upon by supervising pharmacist(s)

• Assist with the completion of various projects and services

• Respond to drug information (DI) requests from prescribers as delegated by supervising pharmacist(s)

• Prepare and present information through various forms of media to other healthcare professionals

• Make and answer phone calls as needed to complete job functions

• Assist the clinical pharmacists including but not limited to the Director of Clinical Pharmacy and the Associate Vice President of Clinical Administration

• Other duties as assigned

Physical Requirement: Hearing: Adequate to perform job duties in person and over the telephone. Speaking: Must be able to communicate clearly to patients in person and over the telephone. Vision: Visual acuity adequately to perform job duties, including reading information from printed sources and computer screens. Other: Requires occasional lifting and carrying items weighing up to 10 pounds unassisted in normal daily activities and up to 30 lbs. for other required work activities. Additionally, requires frequent bending, reaching, and repetitive hand movements (especially if keyboarding and writing), standing, walking, squatting, and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular work shift.
