The Home Health Director of Nursing (DON) Assume primary responsibility for clinical services provided by the Agency. Direct, coordinate and supervise all clinical functions. Maintains compliance with federal and state regulations as they apply to the provision of home visits to patients. Assure compliance with Agency Policy and Procedure. Monitor job performance and completion of evaluations and counseling of clinical staff.

Responsibilities:

Oversight and supervision of clinical activities and nursing and professional staff related to Home Health, including staffing, performance evaluations, education opportunities such as in-service and orientation, guidance, support, and conflict resolution.

Implements infection prevention and control policies and procedures.

Ensures company has appropriate safety and security precautions in place and that they are followed

Understands the company’s Emergency Operations Plan and the role they play in executing it.

Investigates and resolves agency and patient complaints. Protects the patient in the event of a medical/health care error and, as appropriate, assists in error resolution.

Demonstrates thorough knowledge, understanding and ability to perform the functions of the Home Health Agency, including: Intake Service, Patient Assessments and Reassessments, Nursing process, Plan of Care, Discharge Planning, OASIS Data Collection.

Demonstrates thorough knowledge and understanding and keeps up to date on new developments in regard to the accrediting organization standards, federal, state and local regulations, OASIS, and Medicare requirements for home care reimbursement.

Assures quality care by reviewing and assessing each service and through client visits and other measures, as appropriate.

Demonstrates competency in general nursing theory, techniques and practice and delivery of patient care that optimizes patient safety and reduces the likelihood of medical/health care errors.

Demonstrates competence in reporting victims of abuse, National Patient Safety Goals, reporting of medical/health care errors according to established facility standards and state and local laws.

Coordinates referrals.

Assures the development, implementation, and updates of the plan of care.

Ensure coordination of care by all disciplines.

Participate in the review, analysis and appraisal of the effectiveness of the total agency program.

Participate in short- and long-range planning for the agency and implement specific measures for agency growth and increasing market share through education of physicians and other community and referral sources.

Participates in state, local and national organizations, meetings, seminars, workshops and activities relating to the home health profession and health care services.

Develop, implement, review and revise policies and procedures to guide care delivery.

Complete patient documentation on time, as applicable, and ensure direct reports complete their documentation on time.

Organizes and runs staff meetings as needed.

Completes assigned training on time.

Completes annual education requirements.

Maintains patient confidentiality at all times.

Performs other duties as assigned.

Qualifications:

Demonstrate knowledge and skills necessary to provide care to the geriatric and adult population.

Professional demeanor.

Excellent oral and written skills.

Demonstrate organizational and leadership skills.

Proficiency with or the ability to quickly learn the Electronic Medical Record (EMR) and other computer systems.

Education and Experience:

Registered nurse (RN).

Minimum of one year of management/supervisory experience.
