Role and Responsibilities
Case Managers shall coordinate all Provider type (including Secondary Service Providers, District Agency Providers, and Community Providers) services and ensure that families are making meaningful progress toward increasing earnings and achieving housing stability. Case Managers shall be responsible for conducting outreach to referred families and administering the 2Gen approach, engaging families in Provider activities, re-engaging those families who started to participate and subsequently stopped, introducing families to Provider processes, reviewing DHS-administered assessments, developing and amending housing stabilization plans, and helping families overcome barriers to program engagement and participation. Case Managers shall develop trusted working
relationships with assigned families and shall monitor families’ engagement and progress for as long as the families are in the program. Case Managers shall coordinate multiple activities outlined in the housing stabilization plan. Case Managers shall work exclusively (40 hours per week). Case Managers are ultimately responsible for the engagement of families and should work with TEP Providers to identify appropriate education and occupational training programs as well as employment opportunities for families.
Qualifications and Education Requirements
All Case Managers must meet minimum qualification standards. The minimum qualifications for Case Managers are a bachelor’s degree in Social Work, Psychology, Sociology, Counseling, or related service/science or health care related disciplines and one (1) year of experience providing case management services (internship experience applicable). Where necessary and appropriate, a Case Manager without a bachelor’s degree can meet minimum qualifications by substituting at least two
(2) years of work experience in a social service and/or human service field. Certification and/or licensure in relevant discipline (e.g., certified addictions counselor) may also substitute for educational requirements

Knowledge of DC’s Homeless Management Information System
Clinical experience and/or clinical licensure and lived experience preferred.
