Responsibilities of the Nurse Practitioner - Health Risk Assessments:
Conduct comprehensive in-home or remote health risk assessments on Medicare Advantage members
Identify all active and chronic disease conditions to determine physical, mental, and social needs during the visit
Educate and counsel patients on identified conditions and screening procedures
Document findings using the provided iPad
No continuum of care, prescribing, on-call, or follow-up is required
Requirements of the Nurse Practitioner - Health Risk Assessments:
Active and unencumbered Nurse Practitioner license
Minimum availability of 12 hours per week (including one full 8 hour day) is required to qualify
