Responsibilities - Care Coordinator:
Connecting individuals and families to resources available within the community
Responsible for all documentation relative/Data entry
Helping provide services and plans offered to help individuals or families experiencing homelessness or transitioning out
Generating reports and interacting with clients to help assist with services/plans
Strong interpersonal, verbal, and written communication skills with the ability to work independently and in a team environment
Strong attention to detail required and comfortability with Microsoft Excel
Must speak Haitian Creole or Spanish
CPR & First Aid Certificate
Experience working as a case manager/case management, human services, social services, direct support professional/dsp, personal care/pca, caregiver/caretaker, assisted living and home health aide/hha
High school diploma required. Associate's degree preferred
