Job Description

Title: Direct Support Professional

Supervisors: Director of Client Support and Director of Operations; Site Manager, Administrators

Specifications: Prefer Associates Degree in social services, education, or related field. Preferably, one-year experience working with persons with disabilities, blood borne pathogens training, First Aid / CPR certification, and positive crisis management training required.

Duties:

1. Responsible for working cooperatively as a team member to meet the mission, philosophy, and goal of the program.

2. Responsible for assisting, teaching, and supporting clients in the tasks of daily living

3. Responsible for assisting with Peri- Care depending on client needs.

4. Responsible for assisting clients to plan for and access community integration activities.

5. Responsible for assisting and supporting clients to arrange their daily schedules as needed.

6. Responsible for fully implementing clients Individual Support Plans/Person Centered Support Plans, Instructional Individual Support Plans, and Positive Behavior Support Plan (if applicable).

7. Responsible for tracking and keeping data regarding client’s behavior and progress.

8. Responsible for log notations to facilitate communication between shifts.

9. Responsible for assisting and teaching client’s personal hygiene routines, meal preparation, and other independent living skills as determined.

10. Responsible for required record keeping.

11. Responsible for participating in the development of Habilitative Goals and Plan of Care for clients as required.

12. Responsible for maintaining confidentiality of clients regarding disclosure of information.

13. Responsible for informing management of all concerns regarding clients as soon as possible or immediately if there is a grave concern.

14. Responsible for working cooperatively and collaboratively with all support team members including Social Workers, Behavior Specialist, Case Managers, Educators, Guardians, and family members

15. Responsible for working cooperatively and collaboratively with other programs providing services to the client to ensure the best possible experience and outcome for the clients.

16. Responsible for attending all scheduled staff meetings and staff training. This is mandatory.

17. Responsible for lifting and transporting clients as needed. Employees should be able to lift around 25 to 30lbs.
