Health Coach (Nursing)

Responsibilities:

Health Coaching and motivation for individuals and families with specific health conditions for improving health outcomes.

Drawing up personalised care plans for individuals and following up via phone calls.

Travel to the customers place to serve them @ home/ office when required.

Providing culturally appropriate health education on topics related to chronic disease prevention, physical activity and nutrition.

Requirements:

Graduate training in Nursing or relevant courses.

1-2 years' experience in any health industry is preferred.

Excellent communication skills in Telugu, Hindi & English.

Basic computer skills.

High motivation levels and ability to work cordially in teams.
