SPARC Services and Programs is seeking multiple Care Management Extenders to join our Care Management Team. SPARC is dedicated to the mission of Keeping People out of Institutionalized Care and we are launching a new Program, Tailored Plan Care Management to help support our mission. SPARC provides behavioral health services to children, families, and adults across the Piedmont and Mountain Regions of NC.

We are looking for a dedicated Care Management Extenders for our TCM Program. These positions are able to be based out of the Greensboro and Charlotte Offices. Provision of Care Management Extender Services-

Extenders may support care managers in delivering Tailored Care Management by performing activities that fall within the below categories. When an extender performs one of the functions listed below, it may count as a Tailored Care Management contact if phone, video and audio, or in-person contact with the member is made:

Performing general outreach, engagement, and follow-up with members;

Coordinating services/appointments (e.g., appointment/wellness reminders, arranging transportation);

Engaging in health promotion activities (as defined in the Tailored Care Management Provider Manual) and knowledge sharing;

Sharing information with the care manager and other members of the care team on the member’s circumstances;

Providing and tracking referrals and providing information and assistance in obtaining and maintaining community-based resources and social support services;

Participating in case conferences;

Support the care manager in assessing and addressing unmet health-related resource needs.

Qualifications- Care Manager Extenders must have the following qualifications:

· At least 18 years of age and

· A high school diploma or equivalent; and

· Meet one of the following requirements: 1) Be a person with lived experience with an I/DD or a TBI with demonstrated knowledge of and direct personal experience navigating the North Carolina Medicaid delivery system. or 2) Be a person with lived experience with a behavioral health condition who is a Certified Peer Support Specialist. or 3) A parent or guardian of an individual with an I/DD or a TBI or a behavioral health condition and has at least two years of direct experience providing care for and navigating the Medicaid delivery system on behalf of that individual (note that a parent/guardian cannot serve as an extender for their family member). or 4) Has two years of paid experience performing the types of functions described in the “Extender Functions” section below, with at least one year of paid experience working directly with the Tailored Care Management eligible population.

· The Department expects that a range of individuals will be able to meet these qualifications, including, but not limited to: • Certified Peer Support Specialists; • Community health workers (CHW), defined as individuals who have completed the NC Community Health Worker Standardized Core Competency Training (NC CHW SCCT); • Individuals who served as Community Navigators prior to the implementation of Tailored Plans; • Family Navigators, as defined by Trillium Health Resources’ approved In-Lieu of service description; • Parents or guardians of an individual with an I/DD or a TBI or a behavioral health condition (parent/guardian cannot serve as an extender for their own family member); and • A person with lived experience with an I/DD or a TBI or a behavioral health condition

· Ability to engage and build rapport with clients.

· Knowledge of community resources.

· Knowledge of child & adult development.

· Ability to work with a culturally diverse population.

· Ability to make judgment calls and quick decisions in crisis situations.

· Strong oral and written communications.

· Excellent organizational skills and ability to work independently.

· The successful candidate will regularly demonstrate an ability to meet training and paperwork deadlines.

· Demonstrate the ability and willingness to accept feedback - and adapt their own behavior in response to feedback.

· Comfortable with learning to use teleconferencing hardware and software enough to participate in meetings and supervisions with other staff

· Intermediate or advanced knowledge of computer word processing.

· Ability to work flexible hours, carry and respond to a company cell phone daily, and travel as necessary.

· Ability to work well in a team environment required.

· Ability to conceptualize, organize, create and follow through with assigned projects required.

· Must possess/obtain current CPR, First Aid, and Blood Born Pathogens certification

· Possess an understanding of the rules of confidentiality.

· Possess a valid driver’s license, safe and reliable personal automobile transportation, and valid auto insurance policy

· Criminal background checks must meet requirements of service provision standards.

· Ability to climb stairs.
