Salary: $25 - $30 per hour

EverHome HealthCare is a well-established, highly respected Home Health and Home Care agency serving Snohomish, King, and Pierce counties. We are doctor owned and looking to hire an experienced Care Coordinator with a Home Care or Home Health background.

A critical component of the role of the Care Coordinator is to assure that all those who come in contact with EverHome see that our culture is made up of Compassion, Accountability, Results and Expertise, with a strong emphasis on Customer Service. Your primary responsibilities will be to ensure the well-being of our clients and build positive relationships with our clients and caregivers.

Responsibilities

Conduct assessments and evaluations for new and/or potential clients, understanding their specific needs and preferences.

Develop and maintain strong relationships with current clients and their families to establish trust and confidence in our services.

Provide exceptional customer service, promptly addressing any concerns or inquiries.

Check in frequently with client and responsible party and/or regularly visit clients at their homes to assess their well-being and address any issues or improvements needed.

Review and update care plans as needed, considering clients' evolving requirements or change in condition.

Ensure accurate and up-to-date documentation of client profiles and notes.

Provide clear directions for caregivers to reference when working with clients.

Conduct skills verification with new hires and on the job training as needed with caregivers.

Conduct supervisory visits and complete annual performance reviews for caregivers.

Identify opportunities to increase billable hours and expand our service offerings based on the changing needs of clients.

Requirements & Qualifications

Previous experience in healthcare or home care setting.

Excellent customer service and communication skills (both verbally and written)

Customer-focused with a genuine passion for quality care and compassion.

Outgoing, personable, and able to build lasting relationships with clients.

Ability to work independently and in a team.

Comfortable working in the field, conducting assessments, client visits and supervisory visits.

Must have reliable transportation, valid driver's license, auto insurance.

Active CNA/HCA license in the state of Washington

Active CPR and First Aid

Work Schedule:

Our typical office hours are Monday – Friday, 8am-5pm. You will spend approximately 50% of your time in the office. The additional 50% of your time will be in the field doing assessments, plan of care, sup visits, training, coaching, and filling emergency caregiver shifts. Available on call on a rotating basis during the week (including overnight) and weekends.
