FUNCTIONAL DEFINITION

The Service Coordinator will provide advocacy, assessment, outreach, linkage, and assist with the acquisition and coordination of resources and services for the identified clients on their caseload. This position has responsibility for partnering with the client and/or their parent/guardian to identify resources that will build upon the client’s strengths and support and maintain the client’s path towards recovery. This individual will provide enhanced service coordination services to children and adolescents who meet the qualifications of the program to receive community behavioral health services and are at high risk for hospitalization or out of home placement. Work is performed with considerable degree of initiative and independent judgment in accordance with established regulations, policies, and procedures. Position reports directly to the supervisor who provides general guidance regarding program issues.

DUTIES AND RESPONSIBILITIES

Implements and provides services according to the Single Point of Accountability responsibilities:

Be the “go-to” resource for the client served and their family

Assure that there are effective “safety net’ resources for the client served

Clearly communicate to the client and their parent/guardian what they can expect from the system and what the system will expect of them

Assure there is periodic assessment and cross system planning to meet the client’s needs while utilizing their strengths

Prepare for, convene/facilitate service planning meetings and provide follow-up after meetings

Assure there is cross system coordination of services and that services are being provided

Provides culturally competent services with consideration for race, religious, sex, sexual orientation, age and ethnic background

Assist the client in developing and utilizing natural supports

Be a persistent advocate for those served and give feedback on systemic problems

Provide a consistent positive outlook that encourages recovery and full inclusion in the community.

Utilizes and promotes recovery principles in an effort to promote client’s resiliency and recovery.

Maintain billable units monthly reflecting daily activity and turns in case notes to the supervisor on a weekly basis.

Maintains monthly contact with all clients and will meet face to face with clients as needed.

Facilitates and leads all IATM including: being prepared for the meeting with necessary paperwork and background information on the case, elicits all necessary information and brings focus to the meeting, identifies and explains their role within the team and clarifies responsibilities and next steps at the close of the meeting.

Provide ongoing monitoring of treatment plan by attending meetings, reviewing therapeutic case notes that reflect the hours completed, contact with the family and contact with providers. As concerns arise, the service coordinator will provide feedback to provider as well as to the HSAO Supervisor.

Monitor all treatment plans and any concerns with the contracted providers.

Attend and participate in County Interagency Team Meetings

Complies with and adheres to HIPAA confidentiality policy.

Coordinates with families, providers, Children Youth and Families, juvenile probation, educational system and any other community resources as necessary to contribute to client’s success. Attendance at all ancillary meetings with systems involved (court hearings, permanency planning, IEP etc.).

Provide educational advocacy by attending any school meetings and providing technical assistance as needed.

Attends and participates in personal development and remains competent and knowledgeable of current trends, initiatives, resources and services available for clients and families.

Other duties as assigned.

SKILLS AND KNOWLEDGE

Ability to work in a team-oriented atmosphere.

Ability to be a self-starter and creative in identifying solutions to problems; knowing when to inform supervisor when obstacles arise.

Knowledge of Microsoft Office products including Word, Excel, and PowerPoint.

Ability to adapt to and learn new software products.

Ability to work with people from a variety of diverse agencies, backgrounds, resources, and communities.

ESSENTIAL REQUIREMENTS

Possess Act 33, Act 34 and FBI clearance

Have a valid Pennsylvania motor vehicle license and access to a personal vehicle.

Have current auto insurance that meets minimum collision coverage

Knowledge of guidelines and regulations of all levels of Service Coordination Services.

Strong oral and written communication skills.

Excellent leadership qualities.

Ability to work independently.

Full-time position requiring 38.75 hours per week.

EDUCATION AND EXPERIENCE

Master’s Degree in Social Work, Education, or Psychology, OR RN Degree, OR a Bachelor Degree in Social Work, Education, or Psychology with two years’ experience in the behavioral health system. Strong interpersonal and organizational skills are a must.
